Licensed behavioral health specialist with a minimum of 2-3 years ambulatory care experience.
Has experience working with physicians and a working knowledge of behavioral health interventions.
Familiarity with payer demonstration programs, including care management and team-based care.
Knowledge of the Collaborative Care Model (CoCM) program is desired.
Responsibilities include:
identifying and providing behavioral health care management services to GMP Network clients including on-site at the practice or remotely; working collaboratively with the GMPN clinical integration team, physicians, and their staff to help coordinate care, ensuring effective and efficient delivery of services resulting in improved patient outcomes, and reducing health care costs.
Essential Function(s) of the Position:
Assist with identification of clients eligible for behavioral health care management services.
Work with Provider, office staff, and psychiatric consultant to facilitate optimal patient care.
Link patients to community services and support professionals as appropriate.
Educate physicians and office staff regarding patient behavioral health programs, processes and care plan implementation.
In depth knowledge of available resources, contacts and social services.
Review patient needs to assure care team is meeting medical, behavioral, and social needs.
Ensure that patients receive proper care in the most suitable setting by appropriate providers.
Assist with medication reconciliation in partnership with physician.
Work with patients and their families to identify and remove barriers to health care.
Provide information, reinforcement, encouragement, reminders, and follow-up to support members in achievement of their care plan goals.
Other duties as assigned.
Education/Experience Licensed Master Social Worker (LMSW) or Licensed Professional Counselor (LPC) with at least 2-3 years' experience in ambulatory care setting.
Familiarity with behavioral health principles and care models Licensing/Certifications:
Professional clinical license required - Social Work (LMSW) or Professional Counseling (LPC) Valid Michigan Driver's License.
Knowledge, Skills and Abilities:
Understanding of behavioral health processes including Cognitive Behavioral Therapy (CBT), transition of care, ambulatory care, outreach, disease management, family dynamics In depth knowledge of available resources, contacts and social services Experience in providing outreach and/or self- management services to families Knowledge of HEDIS and NQF metrics a plus Knowledge of population health management tools and its function/purpose Willingness to work with metrics to measure outcomes Ability to work with physicians and other health care professionals Recognition of diverse populations and utilization patterns Demonstrated excellent interpersonal communication and presentation skills Ability to work with patient family or other caregiver and, in conjunction with the physician office.
Provide direction and education regarding care plan.
Reporting Relationships:
Position reports to the Director of Clinical Operations.

$50,000.
